Given Dual Nature this is probably fine :)
Mashup of Island and Rage Reflection.
Multi-copy probably isn't printable, as it's too confusing to track which copy is which, but this is the obvious interpretation of "blue rage reflection".
I debated whether it should copy existing creatures, or new creatures, or both, but decided the enchantment was more similar to the original card.
Mashup of Psychic Strike and Phyrexian Reclamation
Pretty nifty for chaining multiple Earthquakes / Blasphemous Acts etc in the same turn.
You're pretty much spot on with your implied modern wording: I'd go for "Until end of turn, if damage would be dealt to target creature, you may instead prevent that damage and add that much white mana to your mana pool."

I tried to include the attacking-creatures-grow and back-and-forth parts of IG and the multiple instant-and-sorceries parts of both cards.
I don't normally like "any player" effects that can be negative for you, but it worked here. I'm not sure if the pump and shrink should both work for both players' attack, that's confusing, but I'm not sure how to do it better.
Well, I can check "You're right, activating sunbeam spellbomb for free is more broken than activating mindslaver for free" off the list of sentences I never thought I'd type :)

Hmm, I was worried about life-gain ones, which is why I set it at 3. I guess I could say that just "Mana and tap costs" are replaced.
A bit good on the Tower of Fortunes cycle, which all cost
to cast and
to activate. Especially amusing on Tower of Eons :)
Is this intended to replace sacrifice costs too? I fear it might, which would make even the humble Pyrite Spellbomb or Aether Spellbomb a pretty terrifying object (and once again goes infinite with Sunbeam Spellbomb... in fact, it goes infinite then draws your deck).
I reckon common is fine for this. Markov Warlord is the uncommon version.
Reminds me of Deepcavern Imp. I think this is fine, and potentially a pretty interesting common in the right set.
Quest for the Holy Relic bypasses both mana cost and equip cost, as well as searching it out. That and it's ridiculously cheap. There is a creature that does all three of those as well - Stoneforge Mystic - but, well, she was banned, for good reason.

Oh, whoops, that's a good point. Yes, without "instant or sorcery" this is getting closer to Dragon Blood and would need rather higher costs accordingly.
Well, it's still admittedly very cheap. Compare the card to Leonin Scimitar to get an idea of how effective this card is.
It also takes "instant or sorcery card". I think perhaps it should lose that bit and just say "Exile target card from a graveyard." I wondered whether it needs "nonbasic" in there, but heck, if you Traumatize yourself or something to get a bunch of basics into the graveyard, then you're still having to tap this on several turns in succession to build up to something really big, so I don't think it's a big problem.
Mahsup of Dragon Blood and Quash
I tried lots of combinations, I'm still not entirely happy with this. It only takes "with the same name" and "from graveyard" from quash.
How should it be costed? It's normally like dragon's blood but a bit less repeatable. But if you can activate it repeatedly it would be very strong. I'm not sure whether to focus it solely on your graveyard or your opponent's or either.
It's also a bit of a "hose flashback" -- I'm not sure if that's good, or should be removed, or made more blatant?
