Oh, Yu-Gi-Oh... This card would definitely take some thought... if you weren't playing a specific combo deck. I don't know how I feel about it, particularly in regards to its expression of luck.
Heart of the Cards - attempting to look lucky without involving actual chance. Probably misses, though.
For Challenge # 013 - intended to be evocative of the luck of the draw.
Otop, probably comes across as far too much of "I know the future and plan perfect strategy" instead, with a side order of cheesy combo du jour. Needs more thinking about.
One in a Million Chances crop up nine times out of ten though :)
Also, how does this work with Dark Confidant? It doesn't tell me what to do with the extra revealed card. Do I just put it back on top of my library?
I like this. I would probably use it. How does it interact with Ponder or Preordain, though? In what order do you put back the revealed cards? If you choose to ignore every other card, you can't put them back in the same order. So this basically doubles the effect of scry, right? Except that you shouldn't be able to put the "extra" cards on the bottom, since you're ignoring them... How would your opponent keep track?
I made Morbid Game, though I'm not sure I like it. I think I'll revisit this challenge later.
For Challenge # 013, in keeping with the fact that each submission has been a different color so far.
I figured I'd use a card type from Innistrad, since it seemed a perfect fit. I'm not really sure if I'm happy with this, but here it is anyway.
You want this to say "If a source would deal", not "Whenever a source deals". That's the sane way to do it anyway, and it also gives you the can't-apply-twice-to-the-same-event rule for free.
I didn't intend for it to be cumulative. I assumed "additional" meant "one more instead", but I'm not sure what the correct templating would be (Pyromancer's Swath suggests it should be "deals that much damage plus 1 instead").
OTOH, there probably should be a cumulative version, perhaps just a shock variant. I'm not sure if it should just be coin-controlled, or if it should be based on the top card of your deck, or something ("~ deals 1 damage to target creature. Clash with an opponent. If you win, repeat the process, and you may choose new targets for the spell."?).
There's a one in eight chance that this will turn into a Lightning Bolt (or better). That sounds rather exciting. I don't think it needs to get more complicated than that.

Chaos of Combat
I quite like Alex's too.
Created for Challenge # 013.
Completely changed from my first idea ("Whenever ~ attacks choose a creature defending player controls at random. You control how that creature blocks this turn.")
I was going to make it more complicated and have the damage be greater or lesser, but decided to start with the simple version. I'm not sure what proportions give a fun, not too complicated, and not too painful feel; I'd rather both players occasionally felt lucky rather than always feeling like they needed to take bad risks.
Created for Challenge # 013. A thematic design thinking about luck, chance, overwhelming odds and suchlike.
Tricky challenge. I created Weighted Dice. There are probably a few other approaches to the problem, but none of them particularly easy.
Edit: I also created One-in-a-Million Chance.
For Challenge # 013. A Krark's Thumb for card reveals. I used to have the Krark's Thumb text on there as well, but it's easier to read without it.
The idea is that a number of cards that use the top of your deck to simulate randomness, such as Kithkin Zephyrnaut or Cruel Deceiver, you now have more control over.
As you pointed out not everyone may knoe /play D&D so I killed that one until a later challenge and decided to play around with the fact that this was challenge 13.
