I think I addressed your concerns somewhat, SadisticMystic.
The loyalty counters are not removed as the result of the transformation or the fact that Nahiri ceases being a planeswalker. Counters aren't removed from a permanent that isn't leaving the battlefield unless an effect explicitly states that they are, even if those counters would normally have zero relevance when placed on the permanent's new type.
Having "Any player may activate this ability" sounds like it's just going to lead to a huge mess, with players repeatedly activating it for X=0 just to try and have it end up on the face they want. A lot of players won't even be aware of the relevant rules (the player who's off turn gets ultimate say on which side it ends up, as long as it's been in artifact form for some portion of the turn).
Would the card maintain loyalty counters as an artifact? It's an interesting way to may use of the tap ability without it costing mana.

I made Venser, the Savior... and then Nahiri, Master of the Stone.
Removed "Artifact" from typing of the transformed side, since it made for a very long type line. Also increased second loyalty cost.

His creature side's second ability is an inversion of Norn's Annex/Ghostly Prison that ends up feeling like Phyrexian blue/white]]].

That -2 is so flavorful and the name of him as a creature is just too awesome, plus the incorporation of paying life in vein of Phyrexian mana. The taxing of the second ability is pretty interesting it's blue and blue but feels green almost (as green comes to mind with cards that can be conditionally blocked, though that thinking probably comes from unblockable being changed to can't be blocked).
Also, you can always add "~ can be your commander."

I'm not sure Multiverse is set up for a non-Planeswalker on the back of a Planeswalker. Also, I'm less of a fan of a planeswalker transforming in this direction, because then they can't be Commanders.
The card should be cast or otherwise normally enter as a planeswalker and when it transforms become another permanent type, not the other way around.

Any reason why the sacrifice trigger is at end of turn? Besides making it effectively cost 15 mana to get any benefit out of it, and 20 to keep it around forever as a rather hard lock?
Why has nobody tried Form of the Eldrazi yet?

Thinking about my original comment more, I think part of what's odd about Form of the Dragon is that it looks Vorthos -- it's all about the flavour of "become a dragon!" But it's actually really Melvin: the whole "your life total becomes 5" isn't representing a generic fantasy dragon, it's specifically imitating how dragon creatures work in Magic. It would be even more Vorthos if it captured the flavour without the "at end of turn your life total becomes 5" trick, but that trick is one of the iconic things about the card.
Trying a simpler solution to an "out" for red.
Green can exile it with Fade into Antiquity or shuffle it away it with Deglamer. Admittedly red is going to have a challenge. The damage prevention should probably also be based on devotion so that red could potentially deal with the permanents providing it.
Green presumably natural... ah, indestructible. Tranquility?
Well, in Red's case... win first? It is an 8-cost card, it is allowed to be hard to beat. Still, all the other "You cannot lose the game" are either creatures or artifacts - or enchantments that have some downside alt-lose condition. Damage turns into cards from deck, or into poison. So maybe that's the way to go?
Um, so what are red and green meant to do about this?
It does seem odd that a card that turns you specifically into one of the Theros Gods - creatures which were noted for being indestructible - in fact serves to make the target far more vulnerable than they were before...
...oh, hey, is this actually a reference to Xenagos? I didn't get that at first.

Technically, yes. But Miracle was built to cast explosive spells for very cheap. This is meant to let you cast utility spells twice. You could also think of it as a less powerful version of Rebound. To be honest, I didn't even notice that Dragons included Rebound until I made this comment and looked up the wording on rebound (go me, for paying attention.) So I was on the right track, but maybe this isn't the best execution.
I would assume that if you got placed into your own library, the game would end, since nobody would be left to draw the cards for you.
Turning a player into a permanent is a ridiculous idea that probably only belongs in silver border.
