Yeah; gaining 15 life is a very odd thing for a black card to be able to do.
How about losing that last entry, and instead having wording like "You can prevent the life loss, that counts as an extra pick"?
I would put the life gain first - it might be weird for players to handle going to negative life during the resolution of a spell.
So... this is for "You gain 15 life." That's not kosher.
The life gain of black is usually tied to destroying something in turn, so "Each opponent loses 1 life, you gain 1 life." would be a more fitting mode.
Note that a target must be legal as you cast the spell and choose targets. Maybe you intent the second and third mode to be a combo? In that case you want the third mode to change to "Destroy target creature if it is tapped. You lose 3 life."
I could also see this progressively trading up resources e. g.
Choose five. You may choose the same mode more than once.
Heck, you could go for "Choose X." that way.

The first ability is fine; the second one is a stretch for green. Maybe give it wither UEOT instead?
It's also quite pushed for that casting cost; the ability to flash in "And your guy dies" is very strong.
This does not fall into hybrid territory.
Centaur Courser is something white usually doesn't get. It's the size where green is supposed to start having more efficient creatures to distinguish between white weenies and green fatties.
Also the flavor text... he died? He's a dead Human Warrior right now?
So, you accidentally used white mana symbols rather than black mana symbols here.

Before anyone gets triggered, I added the reminder text in order to let people know that it's ping also gains life.
I think the question is for a wording that explicitly affects modal spells like Funeral Charm, Austere Command and Mystic Confluence.
There the wording reference in Tahazzar's first comment is useful: The "choices" on a modal spell (bullet points) are called "modes", so "make a choice" becomes "choose a mode".
Choosing a mode is a much more concrete concept in the rules than the abstract making a choice.
I would either way suggest "spell or ability" over "spell or ability".
