You could probably eliminate the extra spaces so that the card is readable without a magnifying glass.

Interesting, though I'm not sure that four black mana is the way to go with this, since it's only going to be played in Mono-B anyway. The first mode seems the weakest, honestly. Maybe some sort of Exhume or slightly bigger Unearth effect might feel a bit better?

Mm. The second part effectively reads

Choose two:

• Raise Dead
• Diabolic Tutor
• Mind Rot
• Moan of the Unhallowed (without the flashback.)

Tutor costs {2} {b} {b} ; Rot costs {2} {b} ; Moan without flashback might cost {b} {b} {b} ?; But Raise Dead costs {b} . And even at that cost, I can't remember any tournament deck ever packing it, despite how ubiquitous the card is.

It would seem that, if you wanted to balance the abilities, then some sort of reanimation spell that costs {2} {b} would be in order. The obvious choices (to me) are Morbid Plunder, Footsteps of the Goryo, or maybe Recover. Or like Mal said, a bigger Unearth would be fine. Or maybe Zombify with an additional loss of 3 life?
