Ooh, I like "Choose X". So for 6 mana, you get a 6/6 flyer, or 6 1/1s with menace (potentially scary with any pump such as Signal Pest), or three 2/4 blockers if you're on the defensive. And for 8 or 10 mana, you spam the board. Cool.

Not really printable in a normal booster set, because of needing three different tokens none of which are vanilla. But I could see something like this happening in a supplemental release that comes with its own tokens, like Commander. Fun.

I always wanted to make a "choose X" card, but never thought of any effects where it seemed balanced. Then I was trying to think of a card for this where it wouldn't just be a creature, and the combination of three different tokens popped into my head.

It's a bit too wordy, I would have left the keywords off the tokens if they weren't already on Demon Advisor.
