It seems we need to clear up the wording again here :)
My best guess is you want it to only trigger from your stuff so you could go with either "Whenever a creature card is put into your graveyard from your hand or the battlefield, ..." which would also trigger if one of your creatures that's currently controlled by an opponent dies, or you could go with "Whenever a creature card is put into a graveyard from your hand or a creature you control dies, ..." which obv only triggers from stuff you have.

Thanks for the tip, went with the second option. Arguably reads worse but probably the version that will cause less calls to a judge.
