I was inspired by keyword counters. Phys rep be tossed aside, let's take notes the old-fashioned way or suffer your own faults for failure to record properly. This was how the Warith group were intended to operate.

Why is this so complicated though? Even setting aside the note-taking aspect, this is fiddlier and wordier than an imprint-style template
