Changed transform condition to ultimately be based on death.
Discovery, unbound, DFC and creature lands all in one card.
On these cards that transform when they die, do we want it to not go to the graveyard? If we dont then I see this wording as being fine but if we do we can have it return from the graveyard transformed in a similar way to Loyal Cathar but immediately.
I've been assuming they don't go to the graveyard. Otherwise, the wording could perhaps be changed to what I did with Unsung Hero, where it exiles and then returns to the battlefield at beginning of end step.

Added "when this permanent transforms into ~" on 2nd side, in line with other similar DFC cards.
Removed inconsistent "You may use this ability only if your discovery is three or greater."-wording. All conditions in a replacement effect belong into the same spot.
This still needs a clause on the front side to actually attach it to the land it is supposed to enchant.
Why the use of +1/+1 counters if the Aura simply could have "enchanted land gets +3/+3 (as long as it is a creature)"?
Hmm you have a point there.
The +1/+1 counters is continueing a theme we have with some other cards where +1/+1 counters is the method, and because of the W/G +1/+1 counters archetype. And the whole point is to make the land a creature, not to buff a land that already is one.

I'm in favor of these being triggered abilities. The wording and gameplay are much more straightforward.
This is now thematically a (two-card?) cycle with Agitated Elemetal.
