Welcome back to you name the card! Just a reminder, that Weakness is a real card that has been reprinted many times, and it looks like this:

It used to see some play, especially to take care of early drops like Savannah Lions. But it's almost been completely obsoleted by Dead Weight nowadays. That said, I could easily see it making an appearance of the common slot of any set, including whatever set this NTC #086 happens to come from.
Blue was chosen to give this card a bit more spice. Weakness decreases power more than toughness, so it feels more blue. You can't decrease toughness in blue, but you still have to pay the
to cast Weakness. Also, I made this require both tapping and sacrificing a creature so that players wouldn't (usually) end up with a huge pile of Weaknesses in their hand.
But what is the name of this card? And what is its creature type?
I feel like just conjuring a Weakness card may be the way to go here.
