Holy cow, this guy is ridiculous. Compare him to Ajani Vengeant and to basically all of the Chandras. A repeatable 4 damage is very strong.
On the flip side, why is his ultimate so similar to his plus abilities? It's very disappointing, especially since it's only one more damage.
This is broken beyond belief, first of all.
The first ability is cute, although it's weird that it triggers on two very different things. But I guess it emphasizes the whole Lightning Helix aspect of the card. That said, maybe one ability can be a 1-damage Helix and the rest do something else. It's really boring that the card talks about dealing or being dealt damage, deals damage and gains life, and then deals damage and gain life.
I'd give you some tips on the numbers, but I feel the overall card needs to change a bit.

This seems a lot fairer now. It looks like a pretty solid design now - my only suggested tweaks would be:
I'll up the loyalty by one I know that the Middle and ultimate abilities are similar, but the goal was that one ability can remove a creature while the ultimate will get you a good amount of life while also helping to make sure that anything that blocks your attack that round dies too.
