Is Memento Mori an additional cost (i.e. kicker), an alternate cost (presumably like evoke), or what?
Memento Mori is an ability cost that the permanent's controller may pay at any time they could cast an instant each turn. Paying the cost multiple times does not increase the number of times the trigger goes off.
Better done as two abilities, then:

At the beginning of each end step, if ~ remembered death this turn, do stuff.
It's also extremely confusing how the current wording has two conditionals in a single sentence and the otherwise supposedly refers to only one of them (or maybe not?).
Based on SecretInfiltrator's suggestion, broke down " Memento Mori
- If ~'s Memento Mori cost was paid, at the beginning of the next endstep put a -1/-1 counter on up to one target creature if a creature you control died this turn. Otherwise, scry 1." into Memento Mori cost being separate from the ability being triggered.
