Barring something like Style Scroll: The Dirty Fighter, creatures can't be attacked. Did you mean this to trigger when the creature blocks?

This ability triggering more than once in a turn is going to be so rare that I don't think you need the sacrifice clause.

I think it's intended to be that you can throw multiple +1's on, but sacrifice in that case.

Wording aside; it's an interesting little card.

The current wording only allows one counter per trigger event.

If you want it to be pumpable, that's perfectly doable. You just need to phrase it either using {x} or as an activated ability, because at the moment this is a once-and-done thing.

The first way would be:

> Whenever ~ attacks or blocks, you may pay {x}. If you do, put X +1/+1 counters on it. If X is greater than 2, sacrifice ~ at EOT.

The second way would be something like Ashling the Pilgrim or Initiates of the Ebon Hand:

> {1}: Put a +1/+1 counter on ~. Activate this ability only if ~ is attacking or blocking. If this is the third time this ability has resolved this turn, sacrifice ~ at EOT.

Note that the second way can be used after the opponent blocks this, while the first has to be used after this attacks but before they declare blockers.

Under what conditions is the creature supposed to have deathtouch? Upon any activation? Upon the third activation? While being sacrificed?

On the third activation. I thought there might be a problem but it still reads fine to me.
