Does is it worth adding a cycle of artifacts producing enemy colored mana like this to the set as a form of mana fixing or is a cycle of common dual lands enough? Ravnica had Signets on its first appearance and Cluestones on the second, so maybe the set does this as a cycle?

Is {3} an appropriate cost for this since you have the potential to get "{t}: Add {u}{r} to your mana pool." While the signets added {u}{r} at a cost of {1} they did it unconditionally and with this cycle you'd need to set up fetish. I don't think fetish is a terribly mechanic to get going, so I'm worried that {3} might be too good, but since it isn't a guarantee {4} seems too expensive.

What if you worded this as follows? Note that this involves reworking Fetish as a keyword, since (from my brief glance at your file), all fetish cards have the exact same working.

Fetish (Whenever a nontoken creature you control dies, if ~ has no fetish, you may exile that card.)
­{t}: Add {u} or {r} to your mana pool. If ~ has a fetish, instead add {u}{r} to your mana pool.

Also, I know I've said this before, but the word "fetish" really is an unfortunate word choice. Players just aren't mature or knowledgeable enough to handle it. There would be giggles in every game.

I'm not fond of the creature having to be controlled by you. Thanks for pointing out that I forget to make the dead creature be a card rather than a token though; that was an oversight of mine. That template would also be much easier on the eyes than what I presently have.

I'm not budging on "fetish." I feel the connotations convey a more unsettling and intimidating atmosphere than other applicable words (or at least the synonyms I've looked at). While this card's name was just pulled as something simple and appropriate for a UR mana rock, others Belt of Heads, Dangling Heart, and (((Sphinxmane Headdress) really resonates with the vibe I'm trying to go for, a word that options such as "effigy" or "talisman," on my opinion, fail to do so.

As a custom set, that's your prerogative. I just wanted you to be aware of the fact that it's not a word that's going to be taken seriously.

A friend pointed out Werewolf: The Apocalypse uses "fetish" in the manner that I do. While I was only ever in one Werewolf game the other players never made jokes about the world, so they must have long ago gotten that out of their system. Then again, that doesn't speak for all groups, but the game does use that term.

I could be underestimating player maturity level based on my personal experiences.

Link, your template is also a lot easier to be immature about than Sorrow's.

That's unfortunately true. I do believe it's easier to understand, though.
