Ugh, I'm really stuck on how to effectively word the first half of the card.
"As an additional cost to cast ~, sacrifice a creature.
Distribute X +1/+1 counters among any number of target creatures, where X is the sacrificed creature's toughness.
Whenever a land card is put into your graveyard, you may shuffle ~ into your library. OR Whenever a land you control is put into a graveyard from the battlefield, you may shuffle ~ into your library."
In the first case, the last ability triggers no matter where the land comes from. In the second, it only triggers when one of your lands dies.
Also replace "shuffle ~" with "shuffle ~ from your graveyard", because otherwise it's not clear whether it applies from the stack or the graveyard.
