Not removing damae during cleanup is a memory issue and should not appear at common/uncommon, probably should not appear at all, ever.

Mmm, wither/infect used -1/-1 counters for their implementation of this. So maybe keep track of the damage instead of a single "has to keep track of damage" counter?

Implementation would be something like "{w}: UEOT target creature takes damage in the form of -1/-1 counters"?

Yeah that's cool, but it doesn't feel super white...

I kind of think that out of all things, the cleanup step should be easy to understand and remember, considering one thing happens once per turn... I suppose it would be hard to remember HOW MUCH damage was dealt, but not if it stayed or not, that is what the counter is for...

I would say 'note toughness' but i don't wanna make an Ice Cauldron lulz.

maybe i just go with a different kind of ability, it's supposed to be controlling and against creatures. I've got one, though it's not as unique.
