Wonderfully thematic! Could probably be pushed power-wise by lowering the equip cost.

Nitpicky but; you can't unequip a creature and then attach something to 'equipped creature'.

I get what you're going for, just the language is a little weird.

Could reword it as ...you control to the creature Transmorphic Polyvest last equipped... or something along those lines. Alternately, you could go this path (which may be open to breaking)

" {0} : Attach another piece of equipment you control to equipped creature and then unequip Transmorphic Polyvest"

So long as it says "Unattach Transmorphic Polyvest from equipped creature", I think "Attach another target equipment you control to that creature" would be clear and accurate.

(It may or may not work with "equipped creature", I'm not sure if that's fixed before you pay the cost or not.)

Do it the way Surestrike Trident or Blinding Powder do it. "Equipped creature gets +0/+4 and has "Unattach Transmorphic Polyvest: Attach another target equipment you control to this creature.""
