(If you have a Birthsign in your opening hand, you may begin the game with it on the battlefield. You can only control one Birthsign at a time.)
Creatures you control have deathtouch.
, Put a star counter on The Serpent: Target creature gains first strike until end of turn. You can't activate this ability if The Serpent has a star counter on it.

Whenever Highway Thug deals combat damage to an opponent, you may discard a card. If you do, draw a card.

Whenever a creature attacks you, sacrifice Shrine to Molag Bal and put a Legendary Equipment artifact named "Mace of Molag Bal" onto the battlefield. It has "Equipped creature gets +3/+2 and has lifelink" and "Equip

Whenever a creature dealt combat damage by a creature you control this turn dies, sacrifice Shrine to Azura and put a legendary artifact named "Azura's Star" onto the battlefield. It has "Whenever a creature dies, put a soul counter on this", and "remove a soul counter from this: Add

Could be a world enchanmtent. :) I like the flavor for this leyline variant.
When it says "You can only control one Birtsign at a time" what does it exactly mean? If some card forces you to put another birthsign onto the battlefield, what is supposed happen? Is it just supposed to stay where it is? Is the one of the buried ala legendary rule?
I think the second ability would be easier to mentally keep track of if the card entered with a star counter and then the cost would be to remove that counter instead of the other way around it currently is.
Regardless, I like the concept and design.
EDIT:
Here's one (bad) idea I came up with:
> The Serpent

> Sorcery — Birthsign
> (If you have a Birthsign in your opening hand, you may begin the game by casting one of them without paying its mana cost.)
> You get three different emblems with —
> • "You can't cast Birthsign spells."
• "Creatures you control have deathtouch."
• "Exile this: Target creature you control gains first strike UEOT."

If that's the case, I'd say just adjust the comp rules to allow things entering the battlefield as the game starts to trigger abilities. It's not like anything else has a competing effect. Actually, to get Karn Liberated to work intuitively, this might already work. ... Yeah, 714.4 already covers this.
If the game begins with it on the battlefield, it entered the battlefield before the game began and thus it wouldn't trigger any ETB effects. I dislike the wording too, but it's necessary.
I don't think you need the convoluted ETB trigger. If you start the game with it, it's still entering the battlefield, so won't the trigger go on the stack at the beginning of the first person's upkeep?
