Triskelavus + Helm of Chatzuk (I ignored the banding and went for 'makes it equipment')
this didn't come out as smooth as I imagined it would, sadly.
What does "Equipped creature gets two +1/+1 counters" mean? If I attach it to a 2/2 and then to a 3/3, do they end up as a 4/4 and a 5/5?
The Helm of Skelavus is mysterious, indeed...
Equip it, it adds two counters. Seemed obvious to me.
And then if you equip it somewhere else and then back again, it gets two more counters?
So it's basically ": Put four +1/+1 counters on target creature you control. Activate this ability only any time you could cast a sorcery and only if you control two or more creatures." I don't think that's broken. (Eventually : six, etc.)
Aye, the repeatable +1 isn't so bad, given the huge equip cost, was my thinking.
If I may suggest templating, then:

Oh, heh, and living weapon is actually rather problematic because the 0/0 germ will die before the triggered ability gets to put the +1s on it.
Hmmm.... I keep smacking my head on that, but keep coming up with a too many sentences to explain something that's otherwise grokable. Does this need Living Weapon? It seems to get in the way, and would require both reminder text and an extra sentence to get counters on the 0/0 token. The reminder text could be removed if this was, you know, Mythic.. but that's probably not a good solution. Meanwhile, the Living Weapon bit doesn't add much to the flavor...
